This award celebrates Primary Care teams that are leading the way in implementing best practices for people living with Type 2 diabetes in their everyday clinical care.
Who Can Apply?
- Any healthcare professional can apply on behalf of their own Primary Care team.
- You may also nominate another Primary Care team—with their consent.
What We’re Looking For
We invite submissions that highlight:
- Innovative approaches to Type 2 diabetes management in Primary Care
- Tangible improvements in patient outcomes
- Sustainable practices that can inspire and be replicated by others
Timeline
- October 1, 2025: Award submissions open
- November 15, 2025: Award submissions close
- January 2026: Awardee informed
- March 11-14, 2026: Award delivery during the ATTD 2026 Award Ceremony
